That Rattle When Your Mercedes Starts? Don’t Ignore It. (M276 & M278 Camshaft Adjuster Failure)

If you drive a Mercedes built roughly between 2012 and 2021 — a C300, C350, C400, C450, E350, E400, SLK350, GLE400, one of the “43” AMG models, or a V8 car like the E550, S550, CLS550, GL450, or ML550 — there’s a sound you need to know about: a metallic rattle for a second or two right when the engine starts, then gone. Runs perfectly the rest of the day. Might not do it again tomorrow. No check engine light.

That sound has a name, and it has a clock attached to it.

The V6 in those cars is the Mercedes M276 — it came in two flavors: the naturally aspirated 3.5-liter and the twin-turbo 3.0-liter (that’s the engine in the C400, C450, E400, GLE400, and the “43” AMG models), and both share this failure. The twin-turbo V8 is the M278. All of them use hydraulic camshaft adjusters on each cam to vary valve timing — you’ll see them called camshaft positioners, adjusters, gears, or VVT sprockets depending on who’s talking. Different names, same part, same failure. Inside each adjuster is a locking mechanism that’s supposed to hold the cam steady until oil pressure builds after startup. As that mechanism wears, the adjuster chatters against its stops for the second or two before oil pressure catches up. That’s your rattle.

Why “It Goes Away After Two Seconds” Is Exactly the Problem

The rattle stopping is what convinces people to ignore it. The car drives fine. There’s no warning light. And because the noise is intermittent — cold mornings, or after sitting a few days — it’s easy to tell yourself it’s nothing.

Here’s what’s actually happening every time it rattles: the camshaft is moving when it shouldn’t be. And on these engines, that movement has a specific, well-documented consequence.

The Exciter Wheel Problem — and How a Rattle Becomes a Camshaft

Each camshaft carries a position sensor exciter wheel (Mercedes also calls it a pulse wheel or trigger wheel) — a toothed ring the cam sensor reads so the computer knows exactly where the cam is. On the M276 and M278, a failing adjuster hammering against the cam can spin that exciter wheel out of its correct position on the camshaft.

This failure mode is heavily documented online — forum threads, shop write-ups, teardown photos. And it changes the repair completely. The exciter wheel isn’t a replaceable part on its own: once it’s rotated out of position, the fix is camshaft replacement. A repair that started as camshaft adjusters — already not a small job — becomes camshafts, on an engine where getting to them means major front-of-engine disassembly.

The engines even telegraph the progression. A worn adjuster often rattles for months with no codes at all. Then timing-deviation codes start appearing — P001177, P001277, P001685, P001885, and P002177 are the common ones, all variations of “camshaft position deviates from specified value.” That’s the computer telling you the cam isn’t where it’s commanded to be. By the time the check engine light is on steady, you’re no longer choosing whether to do the repair. You’re finding out how big it got.

What It Costs — and Why Waiting Multiplies It

We quote off the car, not over the phone, because on these engines the same rattle can come from three different sources — adjusters, timing chain tensioners and their check valves, or a stretched chain — and they carry very different repair bills. Diagnosing which one you have before ordering parts is the whole game. Plenty of M276s have gotten new tensioners and check valves from a general shop, only to keep rattling because the adjusters were the real problem all along.

But to set expectations honestly: the adjusters themselves run around $1,000 apiece, and the labor to open up the front of the engine is substantial — independent shops around the country quote roughly $2,000+ per adjuster all-in, and these engines have four of them (the intake-side adjusters fail most often). That’s the early version of the repair. The late version — spun exciter wheels and camshaft replacement — costs multiples of that.

The math only moves in one direction. Nobody has ever saved money by letting this rattle ride.

Not Just the V6 and V8

We’ve focused on the M276 and M278 because they’re the highest-volume offenders in our bays, but the same camshaft adjuster failure — same rattle, same progression — shows up across much of the modern Mercedes engine family. The M271, the older 1.8-liter four-cylinder, is found in cars like the C250 and SLK250. The M157, the AMG 5.5-liter biturbo V8, powers the E63, S63, CLS63, ML63, and GL63 — yes, the AMG engines do it too, and the parts cost more. The M270 and M274 are the 2.0-liter turbo fours in the CLA250, GLA250, and C300, and the newer M260 and M264 succeeded them in current four-cylinder models.
